STEP 1

SEND THE FOLLOWING INFORMATION TO:
  • Signed release of information from the patient and witness. (Please be sure to
    ensure that the release of information is fully completed)
  • Include in the email body:
    • Your name, credentials, and position
    • Your hospital, clinic, institution
    • Your best direct contact number
    • Your best e-mail address

STEP 2

  • A Tadiso representative will review your message and documentation
  • Tadiso staff will reply via secure e-mail or call the phone number provided
  • If no response within 15 minutes, call: 412-281-3784
    • Note: Weekend response may take 30–60 minutes

  • The dose confirmation may be completed solely by email if no additional information or
    explanation is required.
