Tadiso Dose Confirmation Protocol
—>Note: this is now the only method Tadiso will use for dose confirmations at any time of day, including during business hours
Step 1
- -->Please send these elements to DoseConfirmation@tadiso.org:
- a.) signed release of information for the patient
- b.) your name and credentials/position
- c.) your hospital/clinic/institution
- d.) your best direct contact phone number
- e.) your best direct email address
Step 2
- -->A Tadiso representative will review your message and documentation.
- -->Please allow 15 minutes for the representative to review the documentation and the patient’s chart.
- -->If you don’t receive a response within 15 minutes, you may call the Dose Confirmation Hotline at: 412-495-8490.
Step 3
-->The dose confirmation may be completed solely by email if no additional information or explanation is required.
